Dr. Meredith Troutman-Jordan, PhD, PMHCNS-BC, is an American Nurses Credentialing Center certified psychiatric mental health clinical nurse specialist. Her clinical experience encompasses work in inpatient, outpatient, and residential psychiatric mental health and substance abuse treatment, as well as home health. For the past 20 years, she has primarily focused on working with older adults.

Personalized Self-Management Strategies: Identifying and developing effective self-management strategies tailored for individuals living with dementia and their caregivers to support successful aging.
Evidence-Based Interventions: Striving to find and develop interventions that promote optimal functioning, meaning, and purpose for each individual within the caregiving dyad, recognizing that successful aging is highly individualized.
Desired Outcomes:
- Prevention or slowing of disease and aging-associated complications.
- Improved quality of life for persons living with dementia.
- Reduction of unnecessary healthcare utilization.
- Avoidance of medication complications, injuries, and abuse/neglect of persons living with dementia.
Current Funded Research:
- Investigating the usefulness, feasibility, and acceptability of language, culture-, and ethnicity-specific caregiver resource guides for immigrant nurses and unlicensed assistive personnel or nursing assistants working with persons with dementia.
- Examining home health nurses’ and social workers’ knowledge and self-efficacy related to the recognition and reporting of elder abuse.
- Exploring social determinants of health in relation to nursing students.
