Mandate | Clinical Site Orientation
Information for Compliance with Health and Agency Mandates
Clinical agencies require students to comply with select health and training requirements for the privilege of using the agency and for the safety of the public. The UNC Charlotte School of Nursing requires that ALL students maintain compliance with mandates from its healthcare partners and those specific to the School of Nursing. Any student that is found to be out of compliance will be removed from clinical and will receive an unsatisfactory grade for each day they are out of compliance leading to course failure and therefore, failure to progress in the program. Please note that clinical begins the first week of each semester and includes experiences that occur in healthcare organizations, the community, OR the School of Nursing’s Learning Resource Center.
Students must upload all compliance documentation through a tracking system called Verified Credentials (VC). Students who previously uploaded compliance information in MyRecordTracker by Certiphi will continue to use MyRecordTracker. Students will be given access to VC upon conditional admission. The items below will be tracked on VC or will be requested through a separate process. All items must be completed and kept current. Any items that expire and are not immediately updated in VC will result in removal from the clinical site.
Once a student is given access to VC, this form will be available for download.
Proof of Vaccinations:
- DTP – Diptheria, Tetanus, Pertussis (series of 3 shots)
- Tdap – Tetanus Booster (within the last 10 years)
- MMR – Measles, Mumps, Rubella (series of 2 shots) or positive titer
- Varicella – also known as Chickenpox (series of 2 shots) or positive titer
- History of disease not accepted
- Hepatitis B – Series of 3 shots, positive titer, or declination form
- Once a student is given access to VC, this form will be available for download.
- Flu Shot (required annually)
- 2 Step Tuberculosis (TB) Test or blood test (required initially); TB Screening Form (required annually)
Drug Screen
- Must be completed through Verified Credentials
- No other results will be accepted
Criminal Background Check
Must be completed through Verified Credentials
No other results will be accepted
If you are an international student who has lived in the US for less than 6 years, you will need to pay an additional fee to obtain a Police Clearance Certificate. Please contact your consulate to get instructions on the process and fees. Note, this process should be initiated at least 3 months prior to your practicum/internship.

Bloodborne Pathogens Tutorial and Test (required annually)

Hazardous Medication Training (required annually)

Clinical Site Orientation
Clinical site orientation requirements will vary by organization. Students will be asked to complete orientation modules for a variety of organizations in anticipation of their use as a clinical site. Please follow any instructions provided by the program coordinator, to ensure proper documentation of compliance. Students should keep copies of their completed modules with them at all times. An organization representative OR accreditation official may ask a student or faculty member, at any time, to produce these copies. Moreover, these individuals may ask students or faculty members questions about policies and procedures to ensure adequate understanding. The organizations have the right to remove a student or faculty member from the facility for non-compliance.
